(105) Case Manager – Intensive

Job Description

The person in this position is responsible for the delivery of Intensive case management services to people with severe and persistent mental illnesses, as defined by the state authority. This position primarily provides uniform assessments required by the state authority and the Center, collaborates with individuals served to develop person-centered recovery plans, and provides skills-training, routine case management, crisis interventions and documentation of services delivered in accordance with individual need and Medicaid standards. Participate in QI activities to create a culture of Quality Improvement within the agency/department/program.

The person in this position should provide training in skills that primarily focus on furthering an individual’s independent living, community skills and overall wellness and recovery. Services provided by the person in this position should promote community integration, increased stability within the community as well as an improved quality of life and implementation of self-determination for individuals served, as appropriate. Services provided by this position also include activities, skills-training and case management services designed to address barriers and strengths related to an individual’s recovery and wellness. Services provided in this position would include assisting an individual to gaining and coordinating access to necessary care and services appropriate to the clinical needs. The person in this position would also be responsible for providing crisis intervention services. Assessment, skills-training, case management and support should occur primarily in the community or in vivo, but does not exclude services provided on-site or in the Center.

The person in this position requires an aptitude for working with people who have diagnoses of bipolar disorder, major depressive disorder as well as schizophrenia, schizoaffective disorder and other psychotic disorders; this position also requires knowledge of psychotherapeutic medications as well as evidence-based practices and contraindications in the promotion of recovery for individuals with severe and persistent mental illnesses. This position requires knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.

CredentialsMinimum Qualifications: Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field. Knowledge of mental illness as defined in the DSM-IV is required. Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment

Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred- Spanish
