Job Description

HCS Service Coord.: Responsible for coordinating services for adults and children with a diagnosis of Intellectual/Developmental Disabilities and/or PDD. Duties include assessment, monitoring, service planning, crisis prevention and advocacy. Services provided require good clinical judgment, excellent verbal and written communication skills, ability to work autonomously and professionally with a diverse population, and the ability to submit documentation according to deadline. Job responsibilities are conducted in accordance with HHS Principles, HCS Evidentiary Principles, and Medicaid requirements. This position facilitates the interdisciplinary team process. Must develop and maintain good rapport with the consumers, families, legal guardians agency personnel, and HCS providers. The HCS Service Coordinator be well-organized and proficient in clinical documentation. Must have reliable transportation. Travel is required within the service area daily and within Bluebonnet Trails Community Services area as needed. Work is performed under general supervision of the Service Coord. Supv. with moderate latitude for use of initiative and independent judgment.

CredentialsMinimum Qualifications: A bachelor’s degree or higher with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice. Meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment. Must have a valid Texas Driver's License.

Preferred Qualifications: At least one year experience working with individuals with intellectual/developmental disabilities. Master’s Degree preferred. Demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting. Demonstrated computer and documentation skills. Knowledge of appropriate community resources. Bilingual (English/Spanish). Experience with Medicaid Waiver programs.
Salary Amount$17.13/hr
