Under the supervision of the Center Director and/or the Crisis Supervisor, this position will provide a combination of authority functions to include crisis assessment, referrals to community resources and providers where needed, follow up services and relapse prevention. Services are offered to individuals of all ages. For individuals evidencing crisis or crisis prevention needs who have diagnoses of developmental disabilities (IDD or Autism), the position will provide Service Coordination/Case Management for up to 90 days, coordinating additional provider services that are targeted to address these needs. This care is provided in an effort to reduce the potential for inappropriate psychiatric hospitalizations or incarcerations due to behavioral and/or psychiatric symptoms and, instead, to support individuals with specialized services in their home and community.
Services are provided in the community and in an office setting and will require the ability to develop and maintain partnerships with local law enforcement entities, the medical communities, schools, families, other social service agencies, and within Bluebonnet Trails to facilitate appropriate services and aftercare planning. Interaction with others is necessary on a daily basis and will require an ability to demonstrate appropriate sensitivity for various cultural differences. The ability to get along well with others and work effectively in a fast-paced setting are critical for the performance of these duties. This position requires a flexible work day and involves traveling within the service areas (Bastrop, Lee, & Fayette Counties). Must be able to have and maintain a driving record acceptable to the agency’s insurance carrier. Frequent use of initiative, independent judgment, and the ability to work independently in the field is supported and expected upon completion of training. Supervision is provided on a scheduled and as needed basis. The ability to document in electronic medical records either during the delivery of services or promptly thereafter will be expected.
|Credentials||Minimum Qualifications: A bachelor’s degree or higher with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, or criminal justice. Must have a valid Texas Driver's License.|
Preferred Qualifications: A Master’s degree in a related field, professional license or an intern are preferred. At least one year of case management or service coordination experience working with individuals diagnosed with intellectual or developmental disability or behavioral health conditions is preferred. Knowledge of appropriate community resources and experience with Medicaid Waiver programs is ideal.
