This position is responsible for collecting demographic information and identifying preliminary eligibility status based on a brief psychosocial assessment. This position will be responsible for assessing and differentiating between crisis situations, those meeting eligibility for agency services, and those who require referrals for other providers in the community.
This position provides assessments to adults and youth who have identified symptoms of a possible mental health diagnosis. This position is responsible for completing a psychosocial assessment and gathers information regarding the domains of functioning for the client – medical, social, employment, education, legal, family, housing, substance use and mental health. This information is gathered to make the most appropriate recommendation for treatment and determine if the client meets DSM-V criteria for a mental health diagnosis. Once the intake counselor has completed the assessment and diagnosis and the client meets eligibility for services a treatment plan will be developed in conjunction with the client. Throughout the assessment process and once the treatment plan has been completed the position is responsible for providing brief interventions and motivational counseling for clients that may have difficulty engaging in services. If clients do not meet eligibility the client must be referred to other community resources.
