General Revenue Service Coordination: This position provides services to children and adults with developmental disabilities through assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports and the delivery of advocacy services. The ability to work with families and sensitivity to various agency and interagency personnel to accomplish objectives. The purpose for the position is to empower individuals served to achieve life goals and to live effectively within their families and/or communities. Services provided require good clinical judgment, excellent verbal and written communication skills. Job responsibilities are conducted in accordance with HHS Principles and Regulations. Service Coordinator will have the ability to work autonomously and professionally within a diverse population. The position requires the ability to document electronically using a computer/laptop while providing service coordination activities to document accurately the services provided. The GR Service Coordinator is well-organized and proficient in clinical documentation. Must have reliable transportation. Travel is required within Williamson County daily and within the Bluebonnet Trails Community Service area as needed. Work is performed under general supervision of the IDD Crisis/Continuity of Care Supervisor with moderate latitude for use of initiative and independent judgment.
|Credentials||Minimum Qualifications: A bachelor’s degree or higher with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice. Meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment. Must have a valid Texas Driver's License.|
Preferred Qualifications: At least one year experience working with individuals with intellectual/developmental disabilities. Master’s Degree preferred. Demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting. Demonstrate computer and documentation skills. Knowledge of appropriate community resources. Bilingual (English/Spanish). Experience with Medicaid Waiver Programs.
