Job Description

The Designated Service Coordinator provides service coordination/targeted case management to infants, toddlers and families so that the child will progress in the acquisition and mastery of functional skills in order to reduce or overcome limitations resulting from disabilities or developmental delays. Requirements of early intervention service provision come under the authority of the Individuals with Disabilities Education Act and are regulated by the Texas Department of Assistive and Rehabilitative Services.
Primary Role of Service Coordinator includes overseeing and coordinating the family’s access to resources and needs as well as supporting the family’s understanding of rights and advocacy.
The essential tasks include participation as a member of an interdisciplinary team to evaluate, assess, determine eligibility, develop and monitor treatment plans, document time lines including 28 day and transition, contact families monthly to determine need, complete periodic reviews to change services, document required processes, educate families and community partners, collaborate with physicians and other care providers, maintain required records, and meet the Texas Administrative Code requirement to provide service coordination.
The philosophy of the program is to provide educational, material, and emotional support to families so the child can learn throughout the day in a variety of typical activities, and to support the attachment between the parent or primary caregiver and child. Service Coordination and Targeted Case Management will be provided via telehealth. This position will spend at least 95% of time office based.

CredentialsMinimum Qualifications: Bachelor’s Degree in child development, special education, psychology, social work, sociology, nursing, rehabilitation counseling, or human development or a related field; or an unrelated field with at least 18 hours of semester course credit in child development or human development. Must have a valid Texas Driver's License.
Ability to speak, read, and write fluently in Spanish

Preferred Qualifications: Preferred AND required education, experience, job knowledge, skills and abilities helpful in the performance of this position, as indicated below:
Comprehensive knowledge of typical and atypical development in children birth to three years old
Comprehensive knowledge of childhood conditions that relate to the provision of early intervention services
Comprehensive knowledge of and understanding of team and family dynamics.
Ability to contribute as a leader and facilitator on teams which include parents, caregivers, and other professional staff.
Ability to perform under minimal supervision with considerable requirements for initiative, discretion, independent thought, judgment and self-motivation in matters of significance to families as well as the agency.
Well-developed interpersonal skills, oral and written communication skills, time management/organization skills, and computer skills.
Ability to comprehend and adhere to family-centered, activity-based early intervention practices.
Ability to work sensitively with families from a variety of cultural orientations.
Ability to speak, read, and write fluently in Spanish
Salary Amount$17.57 (stipend for Spanish speaking or fully qualified EIS)/hr
