Job Description

This position is responsible for completing Individual Plans of Care, developing Implementation Plans, supporting methodologies for service provisions, completing all related assessments, coordinating and facilitating of program reviews of the Restraint Risk Assessment, behavior supports, behavior management plans, adaptive aide acquisition, dental service acquisition and minor home modifications acquisition. This position could also assist individuals with the management of personal funds and Medicaid eligibility issues, monitor residential risk areas, coordinate and assist in alternate residential placement needs, provide assistance in identifying individual’s SC special needs regarding accessing medical, social, educational and/or other appropriate support services. This position will monitor all areas of service provisions which support the individual PDP by providing regular contacts with the individual and LAR’s; work with clinical staff to develop and coordinate special needs training for service providers as requested, remain in compliance with all training requirements, meet all required timelines, monitor utilization of funds and revenues; Includes HCS, PASRR, GR programs. May perform other duties as assigned by the IDD Provider Director. At least 50% time will be in direct care services required. Extensive data entry required and travel required.

CredentialsMinimum Qualifications: High School diploma or GED, plus (12) months full-time progressively responsible experience in monitoring and maintaining confidential data, documentation and meeting deadlines. Knowledge of Excel and Word, with record keeping experience and good organizational skills; able to work independently. Must have a valid Texas Driver's License.

Preferred Qualifications: At least one year experience working with individuals with intellectual/developmental disabilities. Demonstrated ability to use good judgment, get along well with others and work effectively in a fast-paced setting. Demonstrated computer and documentation skills. Knowledge of appropriate community resources. Experience with Medicaid Waiver programs.
