The Adult & Juvenile Justice Services Intensive Case Manager (ICM) is responsible for providing person-centered intensive case management and jail diversion services to individuals with severe and persistent mental health disorders who have involvement in the criminal justice system and reside in Fayette County, Texas.
The position reports directly to the Justice Involved Program Director and requires the development and maintenance of effective, professional working relationships with Fayette County legal stakeholders and other community service providers. This is a Monday through Friday, non-exempt, field-based position with flexible hours – no nightshift. The Justice Services ICM regularly meets with individuals in their homes, community, and at Bluebonnet Trails’ clinics to provide psychosocial rehabilitative services. The Justice Services ICM will provide jail diversion and substance use treatment services when needed. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. The caseload size will involve approximately 25 individuals who have been assessed at a 3 or 4 Level of Care. A minimum of 40 monthly hours worked are required to be direct, billable client services with individuals who have Medicaid.
The person in this position must possess an aptitude for working with individuals who have chronic and severe mental health and substance use treatment needs who had been involved in the criminal justice system. Knowledge of psychotherapeutic medications, intensive case management functions, and evidence-based practices in the mental health, substance use, and criminal justice systems are required. This position necessitates skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and treatment, and dual-diagnosis treatment. The person in this position will effectively work independently, prioritize needs during crisis situations, engage in regular meetings and training opportunities, and perform other duties as assigned.
|Credentials||Minimum Qualifications: A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention. Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.|
*If degree is not in a Human Services field, a minimum of 24 hours college credit in a Human Services is required.
Preferred Qualifications: Previous experience in mental health, criminal justice, and/or case management; advanced degree in Human Services field; QMHP certification; Licensed Practitioner (LCDC, LCSW, LMFT, LPC); Bilingual (English/Spanish).
