SUD Crisis Case Manager

Description of Service

Under the supervision of the Director of SUD services, this position will provide a combination of functions to include providing co-occurring psychiatric and substance abuse disorder case management, counseling and follow up services, relapse and prevention and screening and referrals to community resources and providers. This position will provide services in the community setting and the Seguin and Gonzales and clinics and be able to partner with law enforcement, the medical community, schools, families, and social services agencies in a collaborative effort to assist persons in need of services. This position requires a flexible work schedule with days, evenings, and on call services as needed. The person in this position is responsible for assisting crisis case managers by providing crisis assessment, intervention, and screening for hospitalization to individuals experiencing a substance use/ mental health crisis. This position is responsible for assessing an individual’s risk of harm to self and/or others and for providing a preliminary screening for outpatient services to the individual currently in crisis. This positions requires an individual who is able to work both independently and collaboratively.

Eligibility

Minimum Qualifications: A Bachelor’s degree from and accredited college or university with a major in a social, behavioral, or human service field. Knowledge of mental illness and substance use disorders as defined in the DSM-V is required. Must be licensed as an LCDC and have a valid Texas Driver’s License. Must meet the criteria for a QMHP (Qualified Mental Health Professional).
