(1084) IDD Service Coordination Supervisor

Job Description

The Service Coordination Supervisor is responsible for supervising all routine service coordinators as well as unit Subject Matter Experts (SMEs). This includes all hiring activities, providing direction on all client needs such as crisis, casework issues and/or provider issues. Supervisor will assist staff on resolving complaints or conflicts; follow-up on all complaints or conflicts per agency procedure and supervisor direction. Supervisor will provide initial and ongoing training of service coordinators; monitor and track service coordinator performance expectations, including those requirements set forth by HHSC per the Texas Administrative Code. Supervisor will provide unit assistance in the Director’s absence. Supervisor may also provide some service coordination for assigned individuals when unit vacancies are a factor. These responsibilities will entail some travel between agency sites.  If the supervisor is bilingual, supervisor will also be responsible for assisting with translation.

The Supervisor must demonstrate an ability to interact effectively and maintain positive relationships with a variety of agency personnel, both interagency and in the community. This is necessary for satisfactory performance in this position.  Sensitivity to and appreciation of various cultural differences is needed.  Job responsibilities are conducted in accordance with HHSC Principles, HCS and Texas Home Living Evidentiary Principles and Medicaid requirements including the General Revenue program and Community First Choice.

The Service Coordination Supervisor will ensure all individuals in services are assigned and seen routinely to ensure services and supports are in place to meet their needs. All individuals in crisis and transitions in care will need to be staffed weekly and followed through to all required updates in person centered planning. SC Supervisor may participate in interagency planning and CRCG meetings to assist in bridging gaps in services and avoid duplication in services in the communities where individuals receive services. SC Supervisor has a decision-making ability when using Code 11, MAC coding.

Salary: $29.54 – $32.42/hr

CredentialsMinimum Qualifications: A bachelor’s or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. Must have experience providing IDD Service Coordination/Authority function experience, knowledge of person-centered planning, crisis response models and local community resources. Must possess excellent attention to detail and high level documentation skills. Demonstrated ability to use good clinical judgment and be calm and effective in a fast-paced setting. Must meet Bluebonnet Trails MHMRC driving requirements and maintain approved driving status throughout employment. Must have a valid Texas driver’s license and maintain approved driver’s status throughout employment.

Preferred Qualifications: Experience working as a BTCS subject matter expert/lead service coordinator; at least 1 year of experience in caseload supervision and management of a large team. Bilingual. Master’s Degree. Experience with SmartCare and Munis reports.
