As a valued member of the Bluebonnet Community Services Youth and Family services team, this professional position is develops a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, overseeing implementation of the plan, identifying providers of services or family based resources, facilitating child and family team meetings, and monitoring all services authorized for a child’s care.
The Wraparound Facilitator assures that care is delivered in a manner consistent with strength-based, family driven, youth guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.
The person in this position requires an aptitude for working with people who have a mental health diagnoses as well as evidence-based practices in the promotion of recovery for individuals with severe and persistent mental illnesses. This position allows opportunities for skilled professionals to use their knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.

Hourly Rate: $21.16 – $28.41/hour

|Credentials||Minimum Qualifications: Bachelor’s degree in a human services field; comprehensive knowledge of mental health diagnoses. Ability to work as a member of a team within a fast-paced, recovery-focused environment is essential to this position. Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.|
*If degree is not in a human services field, a minimum of 24 college credits in a human services field are required
Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred.
