Job Description

The Care Coordinator collaborates with mental health case managers, substance use providers, intellectual and developmental program service coordinators, primary care physician offices, and other internal and external service providers to promote coordinated care for persons and families in services.  The Care Coordinator provides culturally responsive, person-centered, and trauma-informed services. The position works within and promotes an integrated, holistic model for healthcare at BTCS, which is a Certified Community Behavioral Health Clinic. Services are provided to individuals across an eight-county region; therefore, this position requires an aptitude for researching and connecting with a variety of local resources across a multi-county area. This position is based primarily from home and conducts most work through phone calls and tele-video sessions. Office-based assignments may be included at the discretion of the Director of Care Coordination or Care Coordination Supervisor.

The Care Coordinator utilizes the Bluebonnet Trails Community Services (BTCS) electronic health record to monitor key health indicators and other dashboard information, such as referrals, missed appointments, crisis episodes, and more.  The position may conduct National Outcome Measure (NOMS) assessments to determine risks and needs for individuals with serious emotional disturbances (SED), serious mental illness (SMI), and individuals with SMI or SED and substance use disorders. The Care Coordinator will also complete Adult Needs and Strength Assessments (ANSAs), Child and Adolescent Needs and Strength Assessments (CANS), and Person-Centered Recovery Plans (PCRPs) to address identified needs. Through assessments and monitoring care, this position will support step-down and graduation from services by linking to ongoing community support.

This position reports directly to the Care Coordination Supervisor.

Location: Remote within BTCS service area; onsite work as needed.

Salary: $19.95 – $25.70

CredentialsMinimum Qualifications: Bachelor’s degree in human services field.
Must have a valid Texas Driver’s license and maintain approved driver’s status throughout employment.
Minimum of 6-month case management / crisis intervention / service coordination experience.
Must have skills and experience using integrated computer database programs to access files and document treatment services and the ability to organize and prioritize work assignments.

Preferred Qualifications: At least 1 year of case management/crisis intervention/service coordination experience
Bilingual in Spanish and/or American Sign Language
Experience working in mental health field and/or Local Mental Health Authority
Master’s Degree in a Human Services Field
