The Care Coordinator collaborates with mental health case managers, substance use providers, intellectual and developmental program service coordinators, primary care physician offices, and other internal and external service providers to promote coordinated care for persons and families in services. This position is based primarily from home and conducts most work through phone calls and tele-video sessions. At the discretion of the Director of Care Coordination, the position may require center-based office assignments.
The Care Coordinator utilizes the Bluebonnet Trails Community Services (BTCS) electronic health record to monitor key health indicators and other dashboard information, such as referrals, missed appointments, crisis episodes, and more. The Care Coordinator links individuals and families to healthcare providers, supports follow-up with internal and external providers, conducts routine assessments and surveys, recommends necessary interventions, and facilitates recovery planning.
The Care Coordinator provides culturally responsive and trauma-informed services. The position works within and promotes an integrated holistic model for healthcare at BTCS, which is a Certified Community Behavioral Health Clinic.
The Care Coordinator is connected to a core clinical team to serve approximately 250-300 people. The clinical team may serve more than one county and include more than one BTCS program.
