Job Description | The Population Health Data Specialist is directly supervised by the Population Health Analyst and is responsible for data accuracy and analytics as it pertains to any reportable measure or information coming from the Bluebonnet Trails Community Services (BTCS) system of care. The Population Health Data Specialist will determine accuracy by a process of report and data validation from multiple locations within the electronic health record(s) or within other information sources. This position must assist the Population Health Analyst with the determination of risk for measure noncompliance or data inaccuracy through analysis techniques, efficiently identify data and report anomalies, and communicate these inconsistencies to the Population Health Analyst or additional staff for data reconciliation. Aid in the preparation of data reports, continuous quality improvement activities for off track measures, materials for meeting presentations, and publications for staff and agency use. The Population Health Data Specialist is required to be very detail-oriented, proficient in Microsoft Excel, capable of following measure parameters/specifications, and communicate professionally with all staff. Work within required deadlines to ensure project deliverables are achieved. Provide evaluation of data and processes needs on a regular basis, work independently, and demonstrate efficient use of time in completing all duties assigned. This position will be primarily remote with an occasional on site presence as needed.
