Job Description

The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Parole-Probation Continuity of Care Coordinator (COC) is responsible for providing outstanding person-centered continuity of care and intensive case management services to adults who have a diagnosed severe and persistent mental health disorder, are currently on parole and/or probation, and reside in Burnet County, Texas.

The position reports directly to the TCOOMMI Program Director and requires the development and maintenance of effective, professional working relationships with Georgetown District Adult Parole Officer, Burnet County Community Supervision and Corrections Department and other community service providers. This is a Monday through Friday, non-exempt position with flexible hours – no nightshift or on-call. The person in this position will co-locate with parole and probation as appropriate to ensure continuity of care, in addition to providing home and community-based services. This position involves the option to work remotely to complete only required administrative tasks and/or when telephone or tele video services are deemed appropriate by the supervisor. All remote working must be first approved by the supervisor or member of the executive management team.

The TCOOMMI COC regularly meets with individuals in their homes, community, probation, parole, court, and at Bluebonnet Trails’ clinics to provide case management and psychosocial rehabilitative services for a caseload of 10-15 individuals. Services are intended to stabilize mental health symptoms, reduce risk of recidivism, increase -the individual’s awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. The person hired in this position should be knowledgeable about the Texas Department of Criminal Justice, TCOOMMI Program Guidelines and Processes (PGP), 1.01 and 1.02.

The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and substance use disorders. Knowledge of ACT and Forensic Programs, trauma-informed therapy practices, psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the mental health and criminal justice systems are required. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, and perform other duties as assigned.  Provides community-based support to clinicians in the clinic and field as needed.

Salary: $21.46 – $29.07/hr

$0.50/hour – LCDC

CredentialsMinimum Qualifications: A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention.

Education and experience in behavioral healthcare and/or the criminal justice system.

Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.

*If a degree is not in a Human Services field, a minimum of 24 college credits in a Human Services are required.

Preferred Qualifications: Previous experience working in TCOOMMI, behavioral health, criminal justice, and/or case management; advanced degree in Human Services field; QMHP certification; Licensed Practitioner (LCDC, LCSW, LMFT, LPC); Bilingual (English/Spanish).
