As a Behavioral Health Consultant (BHC), the social worker will work as a member of the Primary Care Clinic’s healthcare team who assists the primary care providers (PCP) in managing the overall health of their enrolled population. The BHC’s goals are to help improve recognition, treatment and management of psychosocial/behavioral problems and medical conditions in the enrolled population. The BHC provides consultation services to all patients referred by the Primary Care Team. The BHC’s primary role is that of a consultant, however, each clinic setting is unique and requires clinical flexibility.

This is an exciting, and dynamic position which allows one to work in our Integrated Health Care clinic in our La Grange clinic. A person in this unique position must feel confident working independently, be efficient at time management, and have wide range of clinical experience, including working with individuals of all ages and families.

Salary: $29.38 – $30.13/hr

CredentialsMinimum Qualifications: Master’s Degree in Social Work from an accredited college or university with a current licensure by the State of Texas as a Licensed Clinical Social Worker (LCSW), current licensure by the State of Texas as a licensed social worker. The candidate will be credentialed through Bluebonnet Trails Community Services. Must have valid Texas Driver’s license and maintain approved driver’s status throughout employment.
