Job Description

Under the supervision of the Director of Care Coordination, the Private Psychiatric Bed (PPB) Care Coordinator is a member of the Care Coordination Team. This position collaborates with partnering psychiatric hospitals to coordinate discharge planning for persons placed at the hospital for inpatient psychiatric care. The PPB Care Coordinator will update local Bluebonnet Trails Community Services (BTCS) treatment teams for persons already enrolled in services, involving key staff in discharge planning and aftercare. The PPB Care Coordinator will also utilize the electronic health record to monitor key health indicators and other dashboard information, such as referrals, missed appointments, crisis episodes, and more. For those not yet enrolled in BTCS services, the PPB Care Coordinator assists with expediting intake and person-centered recovery planning for those who wish to receive BTCS services upon discharge, including competing Hospital Discharge Follow-Up appointments. This position may provide crisis support through face to face crisis assessments to individuals in need. The PPB Care Coordinator will assist with benefits coordination for persons without insurance and screening and referrals to community resources and providers. The PPB Care Coordinator will maintain records and update PPB-related spreadsheets.

This position is based primarily from home and conducts most work through phone calls and televideo sessions. The PPB Care Coordinator may travel to a BTCS location to support individuals with their Hospital Discharge Follow-Up appointment. On Sundays this position will work on site at the Youth Therapeutic Respite Center located in Round Rock, Texas.  The PPB Care Coordinator provides culturally responsive and trauma-informed services. This position works within and promotes an integrated, holistic model for healthcare at BTCS, which is a Certified Community Behavioral Health Clinic.

This position collaborates with law enforcement, the medical community, schools, families, social services agencies and other community stakeholders in a collaborative effort to assist persons in crisis. The individual must possess strong knowledge of the principles, methods, techniques, and practices of working with individuals experiencing a psychiatric crisis and all crisis programs, having the ability to communicate effectively with diverse cultures and socio-economic populations. Must have skills and experience using integrated computer database programs to access files and document recovery services, as well as the ability to organize and prioritize work assignments.

This position requires a flexible workday and the individual must be able to work both independently and collaboratively.

Schedule:  Sunday – Wednesday, evening hours may be included

Salary: $19.95 – $27.45/hr

CredentialsMinimum Qualifications: A bachelor’s degree or higher with a major in social, behavioral, or human service fields including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.

Minimum of 6 months of case management or service coordination experience. Must have a valid Texas Driver’s License and must be able to maintain a driving record acceptable to the agency’s insurance carrier.

Must be willing to work a flexible work schedule to accommodate the needs of individuals and/or their families.

Must have skills and experience using integrated computer database programs to access files and document treatment services and the ability to organize and prioritize work assignments.

Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment.
