Family Directed Provider

Description of Service

Family Directed Provider will facilitate community supported living activities and respite services at the request of the Individuals Family.  Services will be in line with those identified in the Individual’s  Plan.

Rate of Payment

$10 per hour

Areas to be Served

Bastrop County, Burnet County, Caldwell County, Fayette County, Gonzales County, Guadalupe County, Lee County, Williamson County

Eligibility

Successful Provider candidates will be able to demonstrate proof of the following qualifications;
• At least 18 years of age
• Valid Texas Driver’s License or TX ID
• Auto liability insurance
• Clean driving record
• Minimum Education of a High School Diploma or GED
• Pass all criminal, fraud, and employability screenings
