YES Waiver Family Based Alternatives

Description of Service

To provide therapeutic support to a participant and to model appropriate behaviors for the participant’s family, the participant temporarily resides in a home other than the home of his or her legally authorized representative (LAR). The objective is to enable the participant to successfully return home to live in the community with his or her family.

Services may include:

*Age and individually appropriate guidance regarding and/or assistance with the activities of daily living and instrumental activities of daily living (ambulating, bathing, dressing, eating, getting in and out of bed, grooming, personal hygiene, money management, toileting, communicating, performing household chores and managing medications)

*Securing and providing transportation.

*Reinforcement of counseling, therapy and related activities.

*Assistance with medications and performance of tasks delegated by a registered nurse or physician.

*Supervision of the participant for safety and security.

*Facilitating inclusion in community activities, social interaction, use of natural supports, participation in leisure activities and development of social valued behaviors.

*Assistance in accessing community and school resources.

Service Limitation:

Has to be pre-authorized and can be authorized for up to 90 consecutive or cumulative days per service authorization year. There cannot be more than 4 non-related individuals residing in the home.

Rate of Payment

$69.25 per day for foster family $67.98 per day for child placing agency

Bastrop County, Burnet County, Caldwell County, Fayette County, Gonzales County, Guadalupe County, Lee County, Williamson County

Eligibility

*At least one adult who is at least 18 years of age (cannot be spouse or LAR)
*Has to be either a foster family verified with the Department of Family and Protective Services (DFPS) or a child placing agency licensed with the DFPS
*Valid Texas Driver’s License or TX ID
*Pass all criminal, fraud and employability screenings
*Must successfully complete all BBT and State required trainings prior to providing services to individuals
