Job Description | Under the direction of the Youth & Family Service Manager, this position works collaboratively with youth and their families to deliver intensive case management, education related to mental illness and emotional disturbance and to develop coping skills primarily in community-based settings, such as homes and schools. This position conducts routine needs assessments, completing recovery plans, assisting youth and their families with accessing community resources, and initiating crisis supports when needed. This position is supported by an electronic health record and provided in collaboration with a multidisciplinary recovery team made up of psychiatrists, nurses, counselors and family partners who have lived experience with receiving services. Services provided by the person in this position should promote recovery and wellness, community integration, stable living within the community and improved quality of life. Success in this position looks like: possessing a passion and aptitude for working with youth who have severe emotional needs. Knowledge of psychotherapeutic medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field. This characteristics of the person in this position include taking initiative, innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and serving youth that may also have substance abuse needs. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, and perform other duties as assigned with a positive attitude. This is a relief pool position. Hours and schedule are flexible. Credentials | Minimum Qualifications: Bachelor’s degree in human services field; comprehensive knowledge of mental health diagnoses. Ability to work as a member of a team within a fast-paced, treatment setting is essential to this position. Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment. Preferred Qualifications: Master’s degree and/or clinical license in psychology, education, social work or a related human service field; one year work or practicum/clinical experience in intensive case management and/or psychosocial rehabilitative services; bilingual/Spanish-speaking; experience in providing concurrent documentation within an electronic database preferred. |
