(3040) Mental Health Peer Specialist

Job Description

The Peer Specialist will serve as a member of the Intensive Transition Team (ITT) for the State Hospital Stepdown Program as part of a new specialty transitional home designed to support persons dually diagnosed with behavioral health and intellectual developmental disabilities or autism. The State Hospital Step Down program collaborates closely with Health and Human Services and all State Psychiatric Hospitals and works to build successful discharge plans for individuals who have been admitted to the state hospital for over 1 year. The State Hospital Step Down program is designed to step- down/transition individuals with Intellectual and Developmental disabilities with co-occurring mental health diagnosis who are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community-based settings. The ITT will work as an interdisciplinary team to identify, assess, facilitate step-down/transition, house, and provide services and supports to maintain community integration.

As a member of the ITT, services include peer support, Person-Centered Planning, Wellness Recovery Action Planning, Whole Health Action Management, Seeking Safety, and Recovery Dialogue for persons with co-occurring Intellectual and Developmental Disabilities and Mental Health diagnosis. The primary responsibility of peer specialists is to help individuals identify and achieve their own needs, wants, and goals. The Certified Peer Specialist will facilitate and document individual and/or group services at the SHSDP in Bastrop, in the community, at BTCS clinics, and at residential crisis programs such as Diversion Center. The Certified Peer Specialist will work closely with Case Managers, Nursing Staff, Counselors, Management and other BBT staff to ensure continuity of care for the individuals receiving services. The Certified Peer Specialist may be asked to work a variable schedule with some evenings and weekends, as needed.

Salary: $17.30 – $21.05/hr

Applicable Differentials:

Up to $2.00/hr – previous experience in a similar role at a community center

$1.00/hr – 2 years with BTCS

$.75/hour – Bilingual in Spanish/ASL

$200/month Service Delivery Incentive

CredentialsMinimum Qualifications: Lived experience with recovery from mental health challenges and challenges associated with intellectual and developmental disabilities or autism, High School Diploma or equivalent, must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment, must successfully achieve peer support certification within six months of hire.

Preferred Qualifications: Preferred AND required education, experience working with persons with IDD or autism, job knowledge, sills, and abilities helpful; in the performance of this position, as indicated below: Current Wellness Recovery Action Plan (WRAP) Seminar II certificate, Re-entry endorsement, Co-Occurring endorsement, Mental Health Peer Support certifications by Via Hope, Peer Recovery Support Specialist Certification (or equivalent), Whole Health Action Management (WHAM) practitioner, Seeking Safety practitioner, 3+ years experience working as a peer specialist
