(3045) Hospital Step-Down Home Registered Nurse

Job Description

The Registered Nurse (RN) will serve as a member of the Intensive Transition Team (ITT) for the Hospital Stepdown Program. This Hospital Transition Program is designed to step-down/transition individuals who are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community- based settings. The ITT will work as a team to identify, assess, facilitate step-down/transition, house, and provide services and support to maintain community integration.

As a member of the ITT, the RN will collaborate with, as applicable or determined necessary based on clinical presentation and participant preference: state hospital staff, BTCS continuity of care staff, Medicaid Managed Care Organization staff, a peer support specialist, a substance use counselor, the participant, and the participant’s family or legally authorized representative.

Under the direction of the Program Manager and the Director of Residential Nursing, the RN performs responsible professional nursing work or professional administrative nursing work in a 6-bed unit. This position is responsible for the care and treatment of clients in accordance with accepted professional, legal and ethical nursing standards. Primary responsibilities include medication monitoring and administration, individual assessments, laboratory collections and interpretation of lab results, individual and family education regarding medications, health and nutrition, mental illness, and substance use. This position will also be responsible for giving injections, performing venipuncture, administering urine drug screens, and pregnancy tests. This RN will provide support and care to those residing in the home while supervising support staff through education, direction and delegation for other nursing needs and requirements.

The RN is responsible for adhering to all agency policies and procedures and must possess an ability to work within a multi-disciplinary team and have excellent communication skills. This position is Monday-Friday, 8 AM-5 PM with rotating weekly on call duties for the State Hospital Step Down program. This position will cross train to support other programs such as the Bastrop Clinic, Adult Crisis Respite Unit, Youth Therapeutic Respite, and Diversion Center.

Schedule Mon-Fri 8a-5p

Salary: $37.86 – $45.61/hr

Up to $2.00/hr – Previous experience in a similar role with a community center

$.75/hr – Bilingual/ASL

$1.00/hr – Psych Cert**

Up to $4.00/hour – Tenure with BTCS

Rotates into weekly RN on call, $30/day stipend

CredentialsMinimum Qualifications:
1. Graduated from an accredited school of nursing and licensed as a Registered Nurse in the state of Texas with at least 1-year recent nursing experience.
2. Ability to work collaboratively and be part of a multidisciplinary professional team.
3. Pass a standard State and Federal background check
4. Computer literate with working knowledge of Windows/MS office applications.
5. Current CPR certification
6. Excellent written and oral communication skills
7. Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment.

3. Experience in a psychiatric acute treatment unit, psychiatric home care, psychiatric outpatient clinic or any other clinic-based setting.
