Job Description | The position provides continuity of care and intake/access coordination serving persons in Williamson, Burnet, Bastrop, Lee, Fayette, Gonzales, Caldwell, and Guadalupe Counties. IDD Continuity of Care provides care to persons of all ages with IDD or a related condition who are currently or imminently transitioning to or from institutional care or Intermediate Care facility (ICF). This care is provided to increase stability and decrease recidivism during such transitions. ICF placement coordination and authorization includes the Permanency Planning for individuals under age 22 at either SSLC or ICF. May provide support and discharge planning for persons in state hospitals or SSLCs. Duties include providing coordination services to any persons living within the service area, during which transition plans are made to assure that ongoing service needs continue to be addressed through related programs/funding streams. Local travel within the eight county region is required to meet with individuals and their families in the community. Interaction with others is necessary on a daily basis. The ability to work with families, various agencies and interagency personnel to accomplish objectives and the ability to work with families and is critical. The purpose of the position is to empower individuals to achieve life goals and to live effectively within their families and/or communities. Continuity of Care Coordinator will work with individuals who are seeking IDD Services and/or Autism Services. The coordinator will participate in interagency planning and CRCG meetings to assist in bridging gaps in services and avoid duplication in services in the communities where individuals receive services. The Continuity of Care Coordinator will provide intake services to previously identified children and adults with Autism, Developmental or Intellectual disabilities based on the Person-Centered Thinking model through assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports and the delivery of advocacy services. Intake duties relate to completing eligibility screenings for various funding streams (ICF, CFC, Autism and GR), from multiple referring agencies and individuals, referrals to generic resources, admissions to GR, referrals to ICF and CFC services, Autism services and crisis intake support and intervention. Duties also include gathering supporting documents, completing the developmental history, and submitting information to have the individual scheduled for a Determination of Intellectual Disability (DID). IDD Crisis Intake coordination requires frequent use of initiative and independent judgment; sensitivity to various cultural differences; assertive, professional intervention; frequent in person, long-term contact with individuals, families and service providers and the ability to work with minimal supervision. Job responsibilities require traveling within the service area and flexible hours and schedule. Job responsibilities are conducted in accordance with HHS Principles and Medicaid requirements. Credentials | Minimum Qualifications: · An associate degree or higher is required with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice, OR A high school diploma or certificate recognized by the state of Texas as the equivalent of a high school diploma, with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities. · This position serves individuals across multiple settings and requires working knowledge and experience working with individuals diagnosed with Intellectual or Developmental Disability and the IDD service system. · This position requires a demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting. Computer skills and documentation skills are utilized on a regular basis in the performance of assigned duties. Position will be delivered in a combination of work settings, dependent upon the nature of service being provided and/or the need for direct supervision, including agency office, home office, or field (individual’s home or community setting). · Must meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment.
