· Skills Training and Psycho-Social Support: Facilitate skills training to help clients overcome challenges and build the tools needed for a successful recovery journey. This may include face-to-face or televideo sessions in-clinic or in the community.

· Collaboration with Mobile Crisis Outreach Team (MCOT): Work closely with MCOT to ensure seamless continuity of care, from crisis management to recovery support. You’ll also conduct crisis assessments to evaluate risks and facilitate appropriate referrals for further treatment.

· Building Resilience and Safety: Support individuals with high-crisis needs (including those with recent MCOT contacts, crisis line calls, or psychiatric hospitalizations) to build resilience, reduce risks, and develop a sense of empowerment in their recovery process.

Why This Role Is for You:

· You have a deep commitment to mental health recovery and empowering individuals to live fulfilling, independent lives.

· You thrive in a collaborative team environment and enjoy working with other professionals to ensure the best outcomes for those you serve.

· You bring strong crisis management and assessment skills and are motivated by the opportunity to help individuals overcome complex challenges in their recovery journey.
