Job Description | Furthering our Mission to continuously improve the health and independence of the persons we serve and under the supervision of the Clinical Program Manager, the case manager is an essential provider of recovery-oriented case management services, skill building, and medication education The case manager conducts routine psycho-social-emotional needs assessments, completing Person-Centered Recovery Plans, assisting youth, adults, and their families with accessing community resources, providing rehabilitative services, and initiating crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team. Services provided promote recovery and wellness, community integration, independent living within the community and improved quality of life. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. A minimum of 55% of hours worked are required to be direct service. The case manager possesses a passion and aptitude for working with individuals who have chronic and severe mental health needs. Knowledge of psychotherapeutic medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field is critical for success in the position. Effective case management employs innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, clinical documentation skills, and dual-diagnosis treatment. The case manager works independently and within a team, prioritizes needs during crisis situations, engages in regular meetings and training opportunities, and performs other duties as assigned with a positive attitude. Credentials | Minimum Qualifications: Bachelor’s degree in Human Services*, comprehensive knowledge of mental health diagnoses and treatment, ability to work independently and collaboratively within a fast-paced, recovery-focused environment. A demonstrated aptitude to connect with people. Possesses a valid Texas Driver's License and maintains approved drivers’ status throughout employment. *If degree is not in Human Services, a minimum of 24 credits in a human services field. Preferred Qualifications: Master’s degree in Human Services One year’s experience in providing wellness and recovery support services in mental health, substance use and/or crisis intervention and screening Pursuit of clinical licensure in counseling, behavior analysis, family therapy, social work or a related field Advanced licensure Bilingual in ASL or Spanish Experience in providing concurrent documentation Licensure as a Licensed Chemical Dependency Counselor (LCDC) |
