Job Description | Under the direction of the Youth & Family Service Manager, this position will work with youth and their families to deliver intensive case management, skills training, and medication education primarily in community-based settings, such as homes, schools, and the Bastrop ISD Clinic. This position is responsible for conducting routine psycho-social-emotional needs assessments, completing Person-Centered Recovery Plans, assisting youth and their families with accessing community resources, providing rehabilitative services, completing National Outcome Measures (NOMS) and initiating crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team. Services provided by the person in this position should promote recovery and wellness, community integration, stable living within the community and improved quality of life. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. The person in this position must possess a passion and aptitude for working with youth who have severe emotional needs. Knowledge of psychotherapeutic medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field is critical for success in the position. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, and perform other duties as assigned with a positive attitude. Credentials | Minimum Qualifications: Bachelor’s degree in human services field; comprehensive knowledge of mental health diagnoses. Ability to work as a member of a team within a fast-paced, treatment setting is essential to this position. Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment. Preferred Qualifications: Master’s degree and/or clinical license in psychology, education, social work or a related human service field; one-year work or practicum/clinical experience in intensive case management and/or psychosocial rehabilitative services; Bilingual, Spanish speaking; experience in providing concurrent documentation within an electronic database preferred. |
