Job Description | The State Hospital Step Down program provides care in a Transitional Living Home with the 24/7 supports of the Intensive Transition Team (ITT), which the Registered Nurse will serve as a member of. This growing program is designed to step down/transition individuals who are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community -based settings. The ITT will work as a team to identify, assess, facilitate step down/transition, house and provide services and support to maintain community integration. Residents reside in the homes for an average of 6 – 12 months before transitioning to alternative living situations such as independent living, residing with family, or group home living. Our Vision is Healthy and Fulfilled Lives, and as member of the ITT, the RN collaborates with state hospital staff, BTCS continuity of care staff, Medicaid managed Care Organization staff, a peer support specialist, a substance use counselor, the participant, the participant’s family or legally authorized representative to collaborate with the individual towards their recovery goals. Our Mission is to continuously improve the health and independence of the persons we serve, and the State Hospital Step provides critical access to care to support the transition into community living. Critical Impact: Under the direction of the Program Manager and the Director of Residential Nursing, the Registered Nurse performs responsible, professional nursing care and professional administrative nursing work for clients in the program and delegation to the behavioral health support staff who are on site 24/7 in the home. The RN functions as a charge nurse for the program, providing leadership of the medical care to individuals in services and training to colleagues. Key Responsibilities: include medication monitoring and administration, individual assessments, laboratory collections and interpretation of lab results, individual and family education regarding medications, health and nutrition, mental illness and substance use. This position will also be responsible for administering injections, performing venipuncture, collecting and interpreting urine drug screens and pregnancy tests. This RN will provide support and care to those residing in the home while supervising support staff through education, direction and delegation for other nursing needs and requirements. Location: Bastrop TX with travel as needed for training or coverage – Georgetown, Round Rock, Seguin, etc. Hourly Salary Range: $37.86 – $48.61 Differential Detail:

Credentials | Minimum Qualifications: 1. Graduated from an accredited school of nursing and licensed as a Registered Nurse in the state of Texas with at least 1-year recent nursing experience. 2. Ability to work collaboratively and be part of a multidisciplinary professional team. 3. Pass a standard State and Federal background check 4. Computer literate with working knowledge of Windows/MS office applications. 5. Current CPR certification 6. Excellent written and oral communication skills 7. Must have a valid Texas Driver’s License and maintain approved Driver’s License throughout employment. Preferred Qualifications: 1. BSN 2. ANCC Certification 3. Experience in a psychiatric acute treatment unit, psychiatric home care, psychiatric outpatient clinic or any other clinic-based setting. 4. Case Management experience. 5. Experience in working with the IDD population 6. Bilingual-Spanish and or ASL |
