Job Description

The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Transitional Case Manager (TCM) provides person-centered, transitional case management services to adults with severe and persistent mental health disorders who are on parole and/or probation in Burnet and Williamson counties. This role may also provide intensive case management services as appropriate.

This position reports to the TCOOMMI Program Director, and works closely with parole/probation officers, courts, and community providers. This is a full-time, Monday–Friday, non-exempt, field-based position with flexible hours and no nights or on-call requirements.

The TCM manages a caseload of approximately 50–75 individuals, providing services in homes, the community, probation/parole offices, courts, and Bluebonnet Trails clinics. Services focus on stabilizing mental health symptoms, reducing recidivism, increasing engagement with community supports, and supporting successful participation in ongoing behavioral health services.

A minimum of 55% of work hours must include billable and non-billable services to meet productivity expectations. Accurate and timely electronic documentation and compliance with agency and state requirements are essential.

The ideal candidate is passionate about serving justice-involved individuals with serious mental illness and has knowledge of intensive case management, psychotropic medications, substance use treatment, crisis services, community resources, and evidence-based practices. Strong skills in advocacy, coordination of care, assessment, person-centered planning, and dual-diagnosis treatment are preferred.

Hourly Salary Range: $21.46 – $30.71

Differential Detail:

  • Up to $2.00/hour – Previous experience with a community center in a similar role
  • Up to $4.00/hour – Tenure with BTCS
  • $1.00/hr. – Advanced Licensure
  • $0.75/hr. – Bilingual
  • $0.50/hr. – LCDC
  • $1.00/hr. – Masters in related field

CredentialsMinimum Qualifications:
A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention.
Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.
*If a degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field are required.
NCIC and TCIC criminal background checks, substance use screening, and fingerprinting are performed prior to employment.
An official copy of the college transcript is required.


Preferred Qualifications:
Previous experience working in TCOOMMI, behavioral health, criminal justice, and/or case management; advanced degree in Human Services field.
QMHP certification.
Licensed Practitioner (LCDC, LCSW, LMFT, LPC).
Bilingual (English/Spanish).
