(1095) IDD Service Coordinator

This position provides services to children and adults with developmental disabilities through: assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports, and the delivery of advocacy services. The ability to work with families and sensitivity to various cultural differences is critical.  A variety of communication skills are necessary in order to:  interface with individuals served, accurately document service provision, as well the ability to effectively advocate with various agency and inter-agency personnel to accomplish objectives. The purpose for the position is to empower individuals served to achieve life goals and to live effectively within their families and/or communities.

The HCS service coordinator position requires frequent use of initiative and independent judgment; assertive, professional intervention; frequent long-term contact with individuals, families and service providers and the ability to work with minimal supervision. The work environment could consist of a combination of working remotely and in a shared office space, as assigned and deemed appropriate. Job responsibilities require traveling within the service area and flexible hours and schedule. Essential duties include timely completion of comprehensive assessments of individual needs and desires, service planning, referral and linkage, monitoring and advocating on behalf of individuals served, documentation of services provided and interaction with the individual/family served. Job responsibilities are conducted in accordance with HHS Principles, HCS Evidentiary Principles, and Medicaid requirements. The HCS Service Coordinator will participate in interagency planning and CRCG meetings to assist in bridging gaps in services and avoid duplication in services in the communities where individuals receive services. Service Coordinator has a decision-making ability when using Code 11, MAC coding.

Salary: $21.46 – $29.07/hr
