In this role, you’ll be a champion for choice and independence, helping individuals in long-term care facilities access needed services and explore alternatives to institutional living. You’ll work directly with adults who may benefit from specialized services or behavioral health services, guiding them and their families through the process of discovering what’s possible.

What You’ll Do

  • Conduct assessments to determine the most appropriate and least restrictive living options.
  • Coordinate care for individuals with suspected mental illness or intellectual/developmental disabilities.
  • Collaborate with families, nursing facility staff, and service providers to ensure person-centered, recovery-oriented care.
  • Maintain long-term relationships with a small caseload (25–30 individuals), providing monthly face-to-face support.
  • Advocate for community living, making referrals and connecting individuals to the supports they need.

Location: Bastrop, Caldwell and Gonzales Counties

Hourly Salary Range: $21.46 – $30.71

Differential Detail:

  • Up to $2.00/hour, previous experience with a Texas Community Center in a similar role
  • Up to $4.00/hour, tenure with BTCS
  • $.75/hour – Bilingual
  • $1.00/hour – Master’s Degree
  • $.50/hour – LCDC
  • $1.00/hour – Advanced Licensure

CredentialsMinimum Qualifications:
A bachelor’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, or early childhood education/intervention.
*If degree is not in a human services field, a minimum of 24 college credits in a Human Services are required.
At least one year experience, paid or unpaid, with individuals with Intellectual and Developmental disabilities.
Meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment.


Preferred Qualifications:
Previous experience completing assessments and providing case management/service coordination.
Master’s degree in social work, psychology, or counseling.
Bilingual in English/Spanish or ASL.
Experience and knowledge of Medicaid Waiver programs.
Demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting.
Demonstrated computer and documentation skills.
Knowledge of appropriate community resources.
Salary Amount$44,636.80 - $63,876.80/year
