(1501) Case Manager, Relief Pool

Job Description

This position delivers high quality professional recovery-oriented case management and skills training services to people with severe and persistent mental illness.  This position is supported by an interdisciplinary team made up of professional staff including nurses, peers, psychiatrists and others, focused on providing state of the art integrated care.

The person in this position will utilize their expertise to provide training in skills that primarily focus on furthering an individual’s independent living, community skills, and overall wellness and recovery.  Services provided by the person in this position should promote community integration, increased stability within the community, as well as an improved quality of life and implementation of self-determination for individuals served, as appropriate.  Services provided by this position also include activities and skills training designed to address barriers and strengths related to an individual’s recovery and wellness.  Service delivery will occur face-to-face in the community or clinic, with some opportunities for remote work available.

This is a relief pool position.  Hours and schedule are flexible.

Salary: $28.55 – $32.30/hr

Applicable Differentials:

$0.75/hr Bilingual

$1.00/hr Master’s degree

Up to $2.00/hr – previous experience with a community center in a similar role

CredentialsMinimum Qualifications: A Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field.
Must have a valid Texas Driver’s License and maintain approved driving status throughout employment.

Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work, counseling or a related human service field.
