Job Description

The Position: The Bluebonnet Trails Community Services (BTCS) Assertive Community Treatment (ACT) Team Intensive Case Manager is responsible for providing outstanding person-centered intensive case management services as part of the ACT Team to a small caseload of adults who have current or historical substance use treatment needs, a diagnosed severe and persistent mental health disorder, who have experienced multiple hospitalizations. ACT is a model for comprehensive, integrated, community-based services based on a multidisciplinary team to provide psychiatric treatment, social services and rehabilitation. This is an evidence-based treatment initially developed in the 1970s as a ‘hospital without walls” to achieve better functioning in the community for people with severe mental illness. This model was integral in the deinstitutionalization of severe mental illness to allow for community living and is a best practice that allows for individuals to be empowered, have autonomy over their treatment, and reduce recidivism of inpatient psychiatric hospitalization.

This position reports directly to the Behavioral Health Services Manager and requires the development and maintenance of effective, professional working relationships with stakeholders and other community service providers. This position has flexible hours dependent upon the needs of the caseload. This position involves the option to work remotely to complete only required administrative tasks and/or when televideo video services are deemed appropriate by supervisor.

The ACT Intensive Case Manager regularly meets with individuals in their homes, community, hospitals, crisis units, criminal justice settings, and at Bluebonnet Trails’ clinics to provide case management and rehabilitative skills training services. Services are intended to meet people in the community where they are to achieve outcomes to stabilize mental health symptoms, reduce risk of recidivism, increase the individual’s awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. The clinician will be an integral part of a strong team participating in frequent connects to share updates on the recovery process of individuals in service.

The clinician will receive unique and specialized training and professional development, focused on services to a small caseload of individuals that have been hospitalized multiple times as well as a state-of-the-art electronic medical record.

This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self-care, be a delight to work with, and perform duties as assigned with a positive attitude.

Salary: $22.21 – $31.96/hr (plus shift differential)

CredentialsMinimum Qualifications: A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention.

Education and experience in behavioral healthcare, intensive community treatment, and the substance use, abuse, and dependency treatment.

Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.

*If degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field are required.

Preferred Qualifications: Previous experience working with an Assertive Community Treatment program, behavioral health, and/or case management; advanced degree in Human Services field; Licensed Practitioner (LCDC, LCSW, LMFT, LPC)
