(2001) Primary Care Case Manager

Job Description

The Primary Care Case Manager, working within a Rural Health Clinic, is responsible for coordinating and managing the healthcare needs of patients with complex medical conditions and/ or identified social determinants of health concerns. This position ensures they receive appropriate care by collaborating with the healthcare team, ensuring accurate and timely processing of referrals to specialists, coordinating with patients and providers to schedule appointments, verifying insurance benefits, obtaining necessary authorizations, and maintaining detailed records while adhering to RHC regulations and guidelines. This position acts as a key point of contact for both patients and referring providers regarding referral status and next steps, accessing necessary resources, and monitoring their progress while advocating for their needs throughout the treatment plan, all within the framework of the clinic’s policies and guidelines.

Location: LaGrange

Salary: $18.39 – $27.47/hr

Applicable Differentials:

  • Up to $2.00/hr Previous experience in a similar role with a community center
  • $1.00/hr Career Ladder Specialty
  • $0.75/hr Bilingual/ ASL
  • $1.00/hr Tenure with BTCS
