Job Description

The Bluebonnet Trails Community Services (BTCS) mission is to continuously shape and invest in a system of care valued by our communities, designed to improve the health and independence of the persons we serve. Through federal grant funding awarded by the Veterans Administration (VA), BTCS is providing a suicide prevention program for Veterans and their families. The program will provide community outreach and education, screen individuals for suicide risk, coordinate suicide prevention services and VA benefits, assess emergent needs and coordinate referrals or the allocation of general assistance funds, evaluate symptom improvement of participants, and report program data to the VA and BTCS Executive Leadership Team.

Under the supervision of the Veteran Services Grant Project Manager, the Veteran Services Navigator will receive referrals from Veteran Peers and Veteran Treatment Courts to conduct needs assessments, care planning and grant-related services including case management and linkage to community supports, benefits coordination, coordination of financial resources to support employment or gun safety, and referrals to equine-assisted therapy. This position must be able to conduct Adult Needs and Strengths Assessments (ANSA) and Child Needs and Strengths Assessments (CANS) as well as develop Person-Centered Recovery Plans. The Veteran Services Navigator will also conduct mental health screenings each month to assess continued participant eligibility for the program as well as symptom improvement. This position will ideally be filled by a Veteran but may also be filled by the immediate family member of a Veteran. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected.

Salary: $21.46 – $26.45/hr

CredentialsMinimum Qualifications: • Bachelor’s degree in a Human Services field (if degree is not in a human services field, a minimum of 24 college credits in a human services field are required)
• Must have Texas Driver’s License and maintain approved driving status throughout employment
• Ability to work collaboratively and be part of a multidisciplinary professional team
• Must be either a Veteran or the immediate family member of a Veteran
• Must have prior experience with case management, care coordination, and/or benefits coordination
• Must have excellent oral and written communication skills, as well as organizational skills

Preferred Qualifications: • Experience with outreach and representing organizations at events preferred
• Experience conducting clinical assessments and familiarity with VA and other public benefits preferred
• Experience in providing concurrent documentation within an electronic database preferred
• Master’s degree preferred
• Bilingual in Spanish preferred
