Job Description

In alignment with our mission to continuously improve the health and independence of the persons we serve, the PPB Care Coordinator plays a vital role in providing recovery-oriented care coordination services. Under the supervision of the Director of Care Coordination, this position supports individuals by conducting psycho-social-emotional assessments, developing Person-Centered Recovery Plans, facilitating access to community resources, providing support for rehabilitative services, and initiating crisis interventions when needed. All services are delivered in collaboration with a multidisciplinary recovery team and documented in an electronic health record.

This position works closely with partnering psychiatric hospitals to coordinate discharge planning for individuals receiving inpatient psychiatric care. For those already enrolled in services with Bluebonnet Trails Community Services (BTCS), the PPB Care Coordinator provides timely updates to local treatment teams and engages key staff in discharge planning and aftercare. For individuals not yet enrolled in BTCS services, the coordinator facilitates expedited intake and recovery planning, including the completion of Hospital Discharge Follow-Up appointments when appropriate.

Additional responsibilities include:

  • Monitoring key health indicators using the electronic health record, including referrals, missed appointments, and crisis episodes.
  • Assisting with benefits coordination for uninsured individuals.
  • Completing referrals and screenings for community resources and service providers.
  • Maintaining PPB-related records and spreadsheets.
  • Providing culturally responsive and trauma-informed services.

The PPB Care Coordinator also collaborates with law enforcement, medical providers, schools, families, social service agencies, and other community stakeholders to support individuals in crisis. A strong understanding of psychiatric crisis response, crisis programs, and culturally competent communication is essential. This role requires proficiency in electronic documentation systems and the ability to effectively organize and prioritize work tasks. Services provided are designed to promote recovery and wellness, support community integration, and improve quality of life. Accurate and detailed documentation in compliance with agency and state guidelines is expected.

This is a remote-based position that requires a flexible work schedule Monday through Friday, strong independent work skills, and the ability to collaborate effectively within an integrated and holistic healthcare model at BTCS, which is a Certified Community Behavioral Health Clinic. This position operates primarily from home, with most interactions occurring via phone or televideo. Occasional travel to a BTCS location may be required for trainings or office-based assignments, at the discretion of the Program Director.

Location: Remote, must live in BTCS catchment area (Bastrop, Burnet, Caldwell, Fayette, Gonzales, Guadalupe. Lee or Williamson County)

Differential Detail:

  • Up to $4.00/hour – Tenure with BTCS
  •  $.75/hour – Bilingual
  • $Up to 2.00/hour – prior experience in a similar role with a community center in Texas
  • $1.00/hour – Masters Degree in Human Services
  • $1.00/hour – Advanced Licensure

CredentialsMinimum Qualifications:
Bachelor’s degree in Human Services* with a comprehensive knowledge of mental health diagnoses and treatment, ability to work independently and collaboratively within a fast-paced, recovery-focused environment. A demonstrated aptitude to connect with people.
Possesses a valid Texas Driver's License and maintains approved drivers’ status throughout employment.
Minimum of 6 months of care coordination experience.
Must have skills and experience using integrated computer database programs to access files and document treatment services and the ability to organize and prioritize work assignments.
*If degree is not in Human Services, a minimum of 24 credits in a human services field.


Preferred Qualifications:
One year’s experience in providing wellness and recovery support services in mental health, substance use and/or crisis intervention and screening in a case management or care coordination role
Bilingual in ASL or Spanish.
Master’s degree in Human Services.
Experience in providing concurrent documentation.
Salary Amount$43,576 – $60,216/year
