Job Description | The State Hospital Step Down Program is designed to step-down/transition individuals who are diagnosed with Intellectual and Developmental disabilities and are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community-based settings. The Services Manager provides direct supervision to members of the Intensive Transition Team (ITT) and oversees admissions into the program, including the residents of the Transitional Home as well as those residing in the community. The Service Manager is responsible for ensuring the services delivered after enrollment, including oversight of the daily activities within the home. The ITT will plan for and complete the person-centered step-down/transition process. The ITT shall collaborate with, as applicable or determined necessary based on clinical presentation and client preference: state hospital staff, BTCS continuity of care staff, Medicaid Managed Care Organization staff, a peer support specialist, a substance use counselor, the participant, and the participant’s family or legally authorized representative. The Service Manager represents the ITT in court-related processes including written reports and testifying, as approved by the Director of State Hospital Step Down. The ITT shall: 1. Conduct a comprehensive pre-transition assessment for persons at the state hospital who are referred to the program; 2. Provide pre-transition services while the individual is still in the state hospital for up to six months; 3. Develop and facilitate implementation of the Individual Transition Plan (ITP), which must address clinical and housing needs; and 4. Participate in the individual’s care throughout enrollment in the program; The Services Manager participates in the review process for admission to the SHSD program, under the leadership of the Director of State Hospital Step Down or designee. The Services Manager collaborates with internal and external leaders, including contracted Administrator and Assistant Administrator and the owner of the transitional home as it relates to facility issues and repairs. Additionally, the Services Manager authorizes use of program funds, including client support funds. Serves as the Fleet Manager for the Seguin SHSD program. The Service Manager represented the Smithville SHSD program in Continuous Quality Improvement (CQI) activities including internal quality management audits and development and initiation of Corrective Action Plans (CAP) in collaboration with State Hospital Step Down Services Managers for other locations. Credentials | Minimum Qualifications: Bachelor’s degree in the respective discipline, meets BTCS credentialing as a Qualified Mental Health Professional and Qualified Intellectual and Developmental Disabilities Professional. Must have valid Texas Driver’s License and maintain approved driver’s status throughout employment. Two years’ experience providing case management or crisis services to persons with co-occurring IDD and behavioral health diagnosis. Strong computer and communication skills; knowledge of Microsoft Office Programs (Outlook, Excel, Word, Teams & PowerPoint), record keeping, data compilation, good organizational skills. Demonstrated experience with Social Security Disability programs, Medicaid, and Medicare, and demonstrated excellent customer service skills. Preferred Qualifications: Master’s Degree in Human Services; Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW); Licensure as Licensed Chemical Dependency Counselor (LCDC); Bilingual in Spanish, American Sign Language (ASL) |
