Job Description

The Rehab Specialist will serve as a member of the Intensive Transition Team (ITT) for the State Hospital Stepdown Program as part of a new specialty transitional home designed to support persons dually diagnosed with behavioral health and intellectual developmental disabilities or autism. The State Hospital Step Down program collaborates closely with Health and Human Services and all State Psychiatric Hospitals and works to build successful discharge plans for individuals who have been admitted to the state hospital for over 1 year. The State Hospital Step Down program is designed to step-down/transition individuals with Intellectual and Developmental disabilities with co-occurring mental health diagnosis who are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community-based settings. The ITT will work as an interdisciplinary team to identify, assess, facilitate step-down/transition, house, and provide services and supports to maintain community integration.

As a member of the ITT, the Rehab Specialist will collaborate with, as applicable or determined necessary based on clinical presentation and client preference: state hospital staff, BTCS continuity of care staff, Medicaid Managed Care Organization staff, a peer support specialist, a substance use counselor, a supported housing and employment specialist, therapist, nursing team, primary care provider, the participant, and the participant’s family or legally authorized representative.

The primary duties of a Rehab Specialist include:

Conduct a comprehensive pre-transition assessment for persons at the state hospital who are referred to the program, taking intellectual and developmental, medical, substance use and mental health conditions into consideration
Provide pre-transition services while the individual is still in the state hospital for up to six months
Develop and facilitate implementation of the Individual Transition Plan (ITP) and Person Centered Recovery Plan (PCRP), which must address clinical and housing needs
Participate in the individual’s care throughout enrollment in the program, providing case management, skills training, group psychoeducation and crisis intervention services.

This position manages all crises situations for persons in the cross-trains to support the other programs, including the Outpatient Biopsychosocial Approach for IDD Persons (OBI) Program and Mobile Crisis Outreach Team.

Shift: 4 10s schedule*

Options: Sun- Wed 8a-6p or 10a-8p; Wed-Sat 8a-6p or 10a-8p

*Training will occur Mon- Fri 8a-5p

Location: Bastrop

Salary:  $24.23 – $36.48/hr

*$200/month service incentive**

Applicable Differentials:

  • Up to $2.00/hour – previous community center experience in a similar position
  • Up to $3.50/hour – previous crisis experience
  • Up to $4.00/hour – tenure with BTCS
  • $1.00/hour – Advanced licensure
  • .75/hour – Bilingual
  • $1.00/hour, Master’s Degree in Human Services

Shift Differentials:

  • Evening Shift: $2.00/hour
  • Night Shift: $4.00/hour
  • Weekend Shift: $4.00/hour
  • Weekend Night Shift: $6.00/hour

CredentialsMinimum Qualifications: Bachelor’s degree in a social sciences/human services. *Or a minimum of 24 college credits in a human services field and a completed Bachelor's degree are required.
Ability to be credentialed as a Qualified Mental Health Professional/Qualified Intellectual Disabilities Professional
Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment.


Preferred Qualifications: Two years of case management or crisis experience, Master’s degree; pursuing clinical licensure in counseling, social work, or a related field. Bilingual in ASL or Spanish preferred.
