(3041) Mobile Crisis Outreach Team Case Manager – PRN

Job Description

Under the supervision of the Mobile Crisis Outreach Team Program Manager or designee, the Mobile Crisis Outreach Team (MCOT) Case Manager is a member of the elite Guadalupe Hub Crisis Team, providing services in-person and via televideo in Guadalupe, Caldwell and Gonzales Counties. This position is located on site at the clinics withing Guadalupe, Caldwell and Gonzales counites.

The MCOT Case Manager will dispatch to various community locations (i.e. schools, emergency rooms, homes and Bluebonnet Community Services (BTCS) clinics to provide crisis intervention, screening, referral, relapse prevention and follow-up services for children, adolescents and adults experiencing a behavioral health crisis. Follow-up services may include case management, individual and group skills training, Person-Centered Recovery Planning, supported housing/employment, discharge planning and linkage to ongoing services. The MCOT Crisis Case Manager utilizes the most effective and least restrictive approaches and interventions until the crisis is resolved or the individual is transitioned into a more clinically appropriate environment.

Additionally, the MCOT Case Manager cross trains to provide crisis triage, support and intervention services as needed at the Seguin Crisis Respite Unit, State Hospital Step Down Program, Diversion Center, Youth Therapeutic Respite, and other crisis programs.

This position collaboratively partners with law enforcement, the medical community, schools, families, and social service agencies to assist persons in crisis. The MCOT Case Manager possesses strong knowledge of the principles, methods, techniques and practices of working with individuals experiencing a mental health, substance use, and/or developmental crisis, and will have the ability to communicate effectively with persons and families from diverse cultures and socio-economic backgrounds. The MCOT Case Manager utilizes skills and experience using an integrated electronic medical record to access files and document services, as well as the ability to organize and prioritize work assignments. This position requires a flexible work schedule and the ability to work independently, yet collaboratively. This position requires rotating on-call responsibilities, which includes after hours, weekends and holidays. This is an Essential Position which provides services to programs operating 365 days a year as scheduled. The position will provide in-person services during disasters and holidays as scheduled.

Salary: $32.23 – $44.98/hr

Advanced Licensure (LPHA): $1.00/hour

LCDC Licensure: .50/hour

Bilingual: $.75/hour

Master’s Degree in Human Services: $1.00/hour
