(3046) Homeless Prevention Case Manager

Job Description

Under the supervision of the Project Director – Williamson County Homeless Prevention (WCHP), the Homeless Prevention Case Manager will support individuals chronically homeless in Williamson County. The primary goal of this program is to connect individuals to permanent housing through Community First! Village (CFV). This program will also support individuals not interested in or eligible for CFV by referring to Supported Housing or other resources as appropriate and agreed upon by the individual in service.

This professional will respond to referrals from local law enforcement agencies, jails, and private hospitals. The Case Manager will engage with individuals referred to the project to determine interest in and eligibility for housing programs. Responsibilities also include supporting eligible individuals in applying for housing at CFV. In collaboration with the WCHP team, this professional will serve a caseload of up to 15 clients housed at CFV and up to 10 clients in alternate transitional housing. This will include wrap-around care coordination and direct care services to clients in the program (i.e., linkage to clothing, medical care, psychiatric care, and benefits coordination)

This position requires excellent verbal and written communication skills, including completion of needed documentation on the same day of service delivery. The Case Manager can communicate effectively with diverse cultures and socio-economic populations. The ability to collaborate successfully with various community partners is key. The professional utilizes an integrated computer database program to access files and document services. Additionally, this team member has the flexibility to triage tasks and organize the workday to complete needed tasks. This position requires a flexible workday and may support evenings  or occasional weekends. The ability to work independently and collaboratively is essential.

Location: Williamson and Travis Counties

Salary: $25.23 – $37.48/hr (base rate includes Williamson county differential)

Applicable Differentials:

Up to $3.50/hour – Previous crisis experience

Up to $2.00/hour – Previous experience in a similar role with a community center

Up to $4.00/hour – Tenure with BTCS

.75/hour – Bilingual

$1.00/hour – Advanced Licensure

$1.00/hour – Master’s Degree in Human Service

CredentialsMinimum Qualifications:
· Must have excellent oral and written communication skills.
· Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment.
· Must hold a bachelor’s degree in human services or a bachelor’s degree with equivalent coursework in a human services degree.
· Bilingual in Spanish & English

Preferred Qualifications:
· Master’s degree in human services 
· Licensure as a Licensed Chemical Dependency Counselor (LCDC) 
· 1+ yrs experience in a Housing First or Permanent Supported Housing program
