Job Description | The Georgetown State Hospital Step Down (SHSD) Program serves individuals transitioning from the state hospital to a community based residential program. The program is designed to intentionally and effectively assist individuals with reintegrating into the community while promoting resident autonomy and ability to live independently. For 6 -12 months, residents receive person centered recovery planning, relapse prevention, skills training, peer support, medication management and medication education. Support with connection to other community resources, including primary care, dental care, and other outpatient needs are part of the transition process. Critical Impact: The SHSD program began in 2020 as part of an initial pilot program in Texas. The residents transitioning into the SHSD program were previously hospitalized for over one year in the state hospital setting, and provided a new, unique option for care in the community. The SHSD Services Manager manages the day-to-day of how we deliver care to the people we serve, how we develop and support our talented team, and how we collaborate with our community partners. Additionally, the Services Manager leads the communication with our staff and contractors on strategy for maintaining SHSD’s essential operations, including during emergencies and disasters. Credentials | Minimum Qualifications: Bachelor’s degree in Human Services*, comprehensive knowledge of mental health diagnoses, ability to work independently and collaboratively within a fast-paced, recovery-focused environment; minimum of one year of crisis case management experience; possesses a valid Texas Driver's License and maintains approved drivers’ status throughout employment. *If degree is not in Human Services, a minimum of 24 credits in a human services field Preferred Qualifications: Master’s degree in Human Services; pursuit of clinical licensure in counseling, behavior analysis, family therapy, social work or a related field; advanced licensure; Bilingual in ASL or Spanish; experience in providing concurrent documentation, licensure as a Licensed Chemical Dependency Counselor (LCDC). |
