Job Description

The Assertive Community Treatment (ACT) Team Intensive Support Specialist provides outstanding person-centered intensive support services to a small caseload of adults who have current or historical substance use treatment needs, a diagnosed severe and persistent mental health disorder, who have experienced multiple hospitalizations. Services are comprehensive, integrated, community-based services based on a multidisciplinary to provide psychiatric treatment, social services and rehabilitation.

This position reports directly to the Behavioral Health Services Manager and supports the development and maintenance of effective, professional working relationships with stakeholders and other community service providers. This position has flexible hours dependent upon the needs of the caseload. This position involves the option to work remotely to complete only required administrative tasks and/or when televideo video services are deemed appropriate by supervisor.

The Intensive Support Specialist regularly meets with individuals in their homes, community, hospitals, crisis units, criminal justice settings, and at Bluebonnet Trails’ clinics to provide specialized case management and rehabilitative skills training services. Services are intended to meet people in the community where they are to achieve outcomes to stabilize mental health symptoms, reduce risk of recidivism, increase the individual’s awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. The clinician will be an integral part of a strong team participating in frequent contacts to share updates on the recovery process of individuals in service.

The clinician will receive unique and specialized training and professional development, focused on services to a small caseload of individuals that have been hospitalized multiple times as well as a state-of-the-art electronic medical record.

This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self-care, be a delight to work with, and perform duties as assigned with a positive attitude.

Hourly Salary Range: $22.21 – $31.46

Differential Detail:

  • .75/hour, Bilingual
  • .50/hour, LCDC
  • $1.00/hour, Associate or Advanced Licensure
  • $1.00/hour, Two years previous experience
  • Up to $4.00/hour, Tenure with BTCS
  • Up to $2.00/hour, ACT Team differentials

 

  • $4.00/hour, weekend differential
  • $2.00/hour, evening differential

CredentialsMinimum Qualifications:
A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention.
Experience in behavioral healthcare, intensive community treatment, and the substance use, abuse, and dependency treatment.
Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.

*If degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field are required.


Preferred Qualifications:
Previous experience working with an Assertive Community Treatment program, behavioral health, and/or case management.
Advanced degree in Human Services field.
Provisionally licensed (LCDC-I, LCSW-A, LMFT-A, LPC-A).
Salary Amount$46,196.00 - $65,436.00/year
