Network Provider – Preadmission Screening and Resident Review (PASRR) Specialized Services / Independent Living Skills Training (ILST)

Description of Service

The independent contractor will provide direct personal assistance with activities of daily living for adults with developmental disabilities. Includes providing transportation and assisting with life skills training in the nursing facility and in the community.

Independent living skills training — Individualized activities that are consistent with the Habilitation Service Plan and provided in a person’s residence and at community locations, such as libraries and stores. These activities include:

  1. habilitation and support activities that foster or facilitate improvement or maintenance of the person’s ability to perform functional living skills and other daily living activities.
  2. activities for the person’s family that help preserve the family unit and prevent or limit out-of-home placement of the person; and
  3. transportation to facilitate the person’s employment opportunities and participation in community activities, and between the person’s residence and day habilitation site.

Rate of Payment

$16.00/hour individual

Eligibility

At least 18 years of age; high school diploma/GED; copy of DPS driving record and proof of auto liability insurance. At least one year experience providing services to individuals with developmental disabilities. Must use own car and provide transportation for the consumer. Current auto liability insurance required. Good reading and writing skills required. Must pass background check.
