(2068) Relief Pool Wraparound Facilitator

Job Description

This position is responsible for completing a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, overseeing implementation of the plan, identifying providers of services or family based resources, facilitating child and family team meetings, and monitoring all services authorized for a child’s care.

The Wraparound Facilitator assures that care is delivered in a manner consistent with strength-based, family driven, youth guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.

The person in this position requires an aptitude for working with people who have diagnoses of bipolar disorder, major depressive disorder as well as schizophrenia, schizoaffective disorder and other psychotic disorders; this position also requires knowledge of psychotherapeutic medications as well as evidence-based practices and contraindications in the promotion of recovery for individuals with severe and persistent mental illnesses.  This position requires knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.

This is a relief pool position.  Hours and schedule are flexible.

Salary: $28.14 – $29.89/hr

Applicable Differentials:

$0.75/hr Bilingual

$1.00/hr Master’s degree

CredentialsMinimum Qualifications: A Bachelor’s degree from and accredited college or university with a major in a social, behavioral, or human service field. Knowledge of mental illness. Must have valid Texas driver’s license and maintain approved driver’s status throughout employment.

Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred.
