Job Description

This position is responsible for supporting our local transportation services and coordination activities related to the program operation. This position is grant funded and responsible for providing a variety of transportation coordination duties, including coordinating between provider and agency, providing available resources, and program reporting.

Essential job tasks include daily support for transportation services, monitoring all ride request referrals for appropriateness and eligibility, tracking service utilization and service data to report back to funding entities. This position works closely with the Mobility Manager and ensures all individuals with transportation barriers and support needs are addressed. Excel experience, report and data knowledge is required. This position requires good oral and written communication skills as well as a good level of comfort reviewing data and program information.

Other program responsibilities include participation in quarterly transportation service meetings with internal and external partners, completing at least monthly service reports, tracking and submitting data for review, completing basic contact notes in the EHR related to transportation coordination, program billing, service reports in Excel and other duties as assigned.

Other day-to-day duties include but are not limited to answering the phone in a courteous and helpful manner and ensuring individuals in services are connected with the correct staff person. This position must have the ability to work with a variety of agencies and individuals. This position will operate general office equipment and provide clerical duties to data entry; running reports; using multiple agency software, such as SmartCare; Teams, Excel.

This position is 20 hours per week between Monday – Friday, 8:00am – 5:00pm. The person hired will work primarily from home once established. The work location changes will be determined using job task requirements.

This is a part-time position.

CredentialsMinimum Qualifications:
Must have a high school diploma or equivalent.
Must have intermediate experience with Excel spreadsheets and reports and be comfortable managing data.
Applicant must have knowledge of the Bluebonnet Trails geographical service area, including all available transportation resources.
Must have Texas Driver’s License and maintain approved driving status throughout employment.

Preferred Qualifications:
Bilingual.
Previous transportation services work history.
